Dear Patients and Community,
After 49 years of caring for the Fort Worth Community, the practice of Al E Faigin, D.O. and Dr. Nancy Faigin located in Fort Worth, Texas officially closed on April 30, 2025.
Pursuant to Texas State law regarding medical records requests, this document serves as a final request for the transfer of your medical records to ensure continuation of your healthcare with your new physician. This request will remain valid until the completion and transfer of all relevant records.
Requesting Medical Records
WFHA Medical Records Release Policy
3 Years of Medical Records are provided - if older records are needed, please specify the requested time period
Your medical records may include sensitive information. You've already shared some details with me about your health history, I will include this in the records I send. If there are any sensitive topics - such as HIV, substance abuse, mental health, genetic testing, etc - that you'd prefer not to be included, please let me know.
Medical records will be released one time, either to you or your designated physician - if you have requested or received medical records they can not be released again.
Fees:
Pursuant to applicable Texas medical records regulations, the fees for copies of medical records is $25.00 USD for the first 20 pages, with an additional charge of $0.50 USD per page thereafter, plus applicable shipping and handling fees. Please allow a minimum of 2 -3 weeks for processing after payment has been received.
Insurance Companies and Third Party Requests:
Insurance companies and/or other authorized requestor MUST include the Patients Signed Authorization before records can be released.
